Anticoagulation and Immunization Clinic

The goal of the clinic is to maintain each patient’s target level of anticoagulation within an acceptable range to prevent adverse events such as thrombosis and hemorrhage.  Northwest Prescriptions staffs a full-time registered nurse to monitor readings for the Northwest Hospital Anticoagulation Clinic and to administer vaccinations.

Anticoagulation

Northwest Hospital Anticoagulation Clinic operates within a protocol on file with the State Department of Health. Our program is directed by a cardiologist and managed by pharmacists and registered nurses who will:

  • See each patient every four weeks or more if needed
  • Monitor the patient’s laboratory results
  • Review patient’s current medications to check for drug interactions
  • Adjust anticoagulation medication as indicated
  • Communicate the results of this interaction with the patient and
    his/her referring physician
  • Provide education to the patient on anticoagulation therapy

The patient’s enrollment in the clinic begins with a referral from the patient’s attending physician. Upon enrollment, Northwest Prescriptions will contact the patient for an appointment. Prior to the appointment, the patient will report to the Northwest Clinical Laboratory Patient Service Center conveniently located down the hall from Northwest Prescriptions in the McMurray Medical Building on the hospital campus. A finger stick Protime/INR is performed and the result will be immediately sent to the pharmacist in time for the patient’s appointment.

Immunization

  • No prescriptions are required;  it is wise, however, to call ahead and check for availability as certain vaccinations are limited or seasonal.  The links below provide information for each immunization, including availability.
